Do I need a referral?

Will I receive an insurance receipt?

What should I bring?
Bring relevant insurance or claim information and be ready to discuss health history, medication, recent injuries, surgeries and treatment goals.
Do I have to accept every proposed technique?
No. Shawna explains the treatment plan and requests consent. You can ask questions, request changes or withdraw consent at any time.
Which location should I choose?
Appointments are available at 710 Gorham Road in Fort Erie and 20 Willard Avenue in Port Colborne. Available times and locations are shown in the online booking system.
